COVID-19 Vaccine Registration

• Masks are required at all times in our office
• One person is allowed to attend your appointment with you
• Phone appointments are available.
• You will receive a Covid screening upon arrival and before your appointment
• Please reschedule if you have any of the following symptoms:
• Fever or chills
• Cough
• Shortness of breath or difficulty breathing
• Fatigue
• Muscle or body aches
• Headache
• New loss of taste or smell
• Sore throat
• Congestion or runny nose
• Nausea or vomiting
• Diarrhea
